Collaborate with medical providers and admins to arrange medical care and community supports for clients.
Assess client’s current health status using standardized assessment tools and identifying all barriers to achieving their best health.
Develop a nursing plan which is directed toward the optimal resolution of nursing diagnoses and prevention of future problems.
