In the Primary Care setting, the RN CM uses the nursing process and evidence-based practice to work collaboratively with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal and community-based services and disciplines involved in providing care the patient) in developing the patient-driven holistic care plan for life.. Duties include, but are not limited:
Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals
demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources
Supports the providers with their assigned panel of patients to provide effective disease and chronic care management
