The Palliative Care NP is a professional with advance clinical knowledge and competence to function in a variety of roles. The NP works closely with the Section Chief of Palliative Care and the Palliative Care Coordinator to implement the Palliative Care Program. The NP works to ensure high quality practices in practices in the treatment of symptoms at the end of life through the coordination and integration of various nursing, medical, and complementary interventions. The NP duties include, but are not limited to:
Coordinates efforts of the multiple disciplines involved in caring for patients at the end of life in the Palliative Care Unit, the Acute Palliative Care Consult Team, and the Outpatient Palliative Care Clinic
Serves as a consultant and resource person
Triages consults independently, makes medication recommendations, prognostic assessments, and hospice assessments
Manages pain symptoms, runs family meetings, and coordinates care
Develops and implements educational programs
Coordinates with the VISN Palliative Care
Serves as a liaison to community hospice and internal and external marketing of Palliative Care Consult Services
